Description
Complete the following steps to start the HSI Instructor process:
- Complete the HSI Application for Authorization (form must be completed by new and renewing instructors.)
- Send completed form, along with all valid, professional certificates that have been checked off in Steps # 3 and 4 on the HSI Application for Authorization, to LifeForce USA, via email (preferred method) or mail.

If you have not met the teaching requirements, you must provide proof of current equivalent instructor certification accepted by HSI.
